General guidance notes

This form (available on the right hand side) must only be completed if:

  • you have discussed the vacancy you are interested in with a Jobcentre Plus member of staff; or
  • the vacancy instructs you to do so.

You must save a copy of this form to your computer before you use it. It must not be completed on-line.

Either a Jobcentre Plus member of staff or the vacancy itself will tell you how and where to send your completed form. Remember to keep a copy of your form, as it will be a useful reminder if you are invited to an interview.

General notes on completion of a Jobcentre Plus application form

  • Answer all questions giving specific information that is relevant to the job you are applying for, avoiding the use of 'not applicable' (n/a).
  • If you have any difficulty completing this application form, your local Jobcentre Plus office can assist you.
  • Complete the form on your computer, print it out before signing it and send it to the address shown on the vacancy details or given to you by a member of Jobcentre Plus staff.
  • Do not email the form to either the employer or Jobcentre Plus unless instructed to do so by the employer or a member of Jobcentre Plus staff. Completed application forms must be returned in person or by post to either the Jobcentre or the employer – you will be told which.
  • For overseas vacancies only you must have a valid passport or EEA ID card. You may enclose a CV instead of completing sections 6 & 7. If copies of official documents or certificates are requested, DO NOT include originals.

Guidance for completion of this application form

Vacancy Details: Ensure that you have all the details from the Jobcentre, telephone job-matching service or the vacancy before completing.

Section 1-4: Complete with factual information.

Section 5: Give details of driving licences held ie. Motorcycle, car, HGV, PSV. Also declare details of any points on your licence.

Section 6: Give details of previous employment starting with the most recent first. You should give as much detail as possible about your duties in each job.

Section 7: Give details of schools, colleges, universities or other training establishments attended starting with the most recent first. You should list any examinations taken and any training courses attended, as well as grades and results awarded.

Section 8: You must complete this section if you are applying for an overseas vacancy.

Section 9: This is a very important section on the form, it gives you the opportunity to sell your skills to the employer. You should include any relevant information that you feel makes you particularly suitable for the job you are applying for. This may include previous experience, voluntary work, hobbies, and other relevant qualifications. If you have no formal qualifications, you might want to include anything you were good at in school.

Section 10: You must complete this section if there is an age limit on the vacancy to confirm that you meet the age requirement.

Section 11: This section asks whether you consider yourself to be disabled as specified by the Disability Discrimination Act 1995. Under the Act, a person is considered to be disabled if they have a physical or mental impairment that has a substantial and long-term adverse effect on their ability to carry out normal day-to-day activities. Also mention in this section if you require any specific arrangements to be made should you be invited for an interview.

Section 12: You should provide the names, addresses, telephone numbers and occupations of two people who will act as character referees for you. This is usually a previous employer and someone whom you have known for at least 5 years such as a family friend or neighbour, but not a relative. Please check with your referees that they are willing to provide a reference for you before entering their name on your application form.

Section 13: Before completing this section you must print the form from your computer. Then sign, date and print your name in the boxes provided.
